At Moorfields we want all our patients to be able to make informed decisions about the care we provide for you. This means we will discuss your treatment options with you and help you understand what that means for you.
As a reminder of this conversation for your records, and to ensure you are happy to proceed with the clinically prescribed treatment for you, we are using a tool called Concentric.
This tool allows us:
- Share information about what’s involved in your treatment
- Tell you why we’re suggesting this treatment for you
- Explain any alternatives to the treatment you’re electing to have
- Outline what you might expect as a natural course of the prescribed treatment
- Get your approval to proceed with planned treatment or surgery
How you will be contacted
We can send you information on the Concentric application via either text (sms) message and / or email depending on the contact details you have shared with us.
Text message (SMS)
This is what a text message (sms) from Concentric might look like when we send one to you.
- It will indicate that the message has come from Concentric.
- It will be addressed specifically to you by the name we hold for you on record.
- It will always come from a link that starts https://patient.concentric.health.

As a security measure the link will ask you for your stated name and your date of birth.
Once you’ve accessed your information, you can respond to us directly with your agreement to proceed with this treatment.
Email message
This is what an email from Concentric might look like when we send one to you.
- It will always be from “Moorfields Eye Hospital NHS Foundation trust (via Concentric)” and it will always be from notifications@concentric.health.
- It will only ever be sent to your email address, the one we have on record for you, and to no other email addresses.
- It will always be personally addressed to you by the name we hold for you on record.
